Cancellation of Payment and Refund PolicyCancellations of online payments can be made by request when an alternative payment is offered. If you need to cancel an online payment, please contact the Office Manager at (952) 941-7393. Payors are responsible for all cancellation charges. Refunds on online payments made in advance of treatment/delivery of services will be made in the event there is a credit on your account, and in the amount of the credit on the account after the online payment is applied. Such refunds will be processed within 21 days period. Online payments to reserve the Operating Room are refundable if there is a credit on your account, or if the appointment is cancelled seventeen (17) days before the appointment date. Please note that within 16 days of the appointment, the deposit becomes non-refundable. |
